3) Coach’s Name
Important: Your membership sanction number is the order number that you’ll receive upon completion.

Participation in AFA Slow Pitch Tournaments requires an AFA Slow Pitch Team Membership for the 2023 Season. The membership fee is $30.00 per calendar year per team.
